Clinical and electrophysiological study of a patient with Kleine-Levin syndrome.

A recurrent hypersomnia syndrome, with hyperphagia and visual and auditive hallucinations is described in a girl from 8 to 13 years old. It seems that this sleep and behavioral disorder was developed after a tonsil infectious disease with high fever. The crisis started afterwards, with intercrisis periods of 1 to 4 weeks, lasting each attack 7 to 15 days The results of psychiatric, pedagogical, psychometric, imagenologic studies (nuclear magnetic resonance and computerized axial tomography) and neurophysiological (conventional and quantitative EEG and P300 event related potential), were analyzed and discussed. Only conventional and quantitative EEG results showed an abnormal slowness (theta increased and alpha decreased activity) in both conditions, with and without hypersomnia crisis. Illness course has been satisfactory since hypersomnia crisis number and intensity has progressively and spontaneously decreased. Since one and a half years ago, the patient remains without clinical manifestations. EEG abnormal patterns have remained without changes. The other studies, including P300 waves parameters and imagenological tests have always been normal. Results are discussed on the basis of a brain electrical activity disorder and a Kleine Levin syndrome developed in a sex and age not frequently observed in clinical practice.
