The psychiatric reform constitute one the major changes of the Spanish Health System during the last decade. It is outstanding the lack of opportunity cost or outcome research studies that may help to stablish some guidelines for future plannings. On the other hand, it is necessary to provide a critical view of such process free from dogmatiory. We provide a review of this issue from an international perspective where we introduce a conceptual background and identify some key problems which may be relevant for the assessment, as well as for planning health policy and needs in the future.
